The final chapters of a life journey can be one of the most difficult for an individual and their family to endure. Through the many Acclaim Health programs, our dedicated nurses and trained volunteers provide in-home medical care and emotional support for individuals living with life-threatening illness. Our bereavement services also support individuals experiencing grief and loss through the death of a loved one.
