Tele-Touch Program

Imagine waking up in the morning and not knowing when your next phone call would be, or without knowing that someone out there cares about your happiness and well-being.

Through the Tele-Touch program we provide reassuring telephone contact to isolated seniors and physically challenged adults. Volunteers get in touch with individuals through a prearranged scheduled of telephone calls for friendly conversation, security and reassurance.

Friendly Visiting Program

Not all seniors are active or have the support of family and friends nearby to help out with their daily activities.

The Friendly Visiting Program provides regularly scheduled social visits or outings to frail or isolated seniors and physically challenged adults that assist them to live as independently as possible in their own homes.

Hospice/Bereavement Program

Imagine the doctor telling your husband that he has an inoperable brain tumour. Then imagine trying to take care of him as this disease takes over.

We offer the Hospice Visiting Program for individuals faced with life-threatening illness and for their families. Through in-home visits, specially trained Volunteers provide support to individuals who have chosen to stay in the comfortable and familiar environment of their home.

Our Bereavement Services provide support to people experiencing grief and loss through the death of a loved one. We provide in-home visits, one-on-one support and an in-house resource centre with a lending library.

Special Steps Program

A lot of seniors do not have family or friends nearby that they can go out and be “social” with or engage in some physical activity.

Special Steps provides support for at-risk seniors experiencing memory loss. This program is designed to bring regular social and physical activity and stimulation through scheduled walks and/or in-home visits.

Alzheimer Services

Acclaim Health provides support and assistance to clients and their families dealing with a diagnosis of dementia. With one in 13 Canadians over age 65 suffering from Alzheimer Disease or a related disorder, socialized services and support are more important than ever.

We provide day programs in both Burlington & Oakville, as well as various education and information sessions across Halton for families dealing with a diagnosis.

Home Health Care

Nursing Services

Acclaim Health Nursing professionals offer an array of services that provide quality care and support in the comfort of people’s homes. Care is provided 24 hours a day, seven days a week.

Home & Personal Support Services

Acclaim Health Support Services enhance an individual’s quality of life by providing them with the assistance and support needed to make them comfortable in their own homes. Acclaim Health hires only professional, certified Support Workers who provide a wide range of services including bathing, skincare, dressing and feeding.
