Acclaim Health offers Hospice Visiting & Bereavement Support Programs for individuals facing life-threatening illnesses or who have recently experienced the loss of a loved one.

Individuals can refer themselves or be referred by family members, physicians or other community agencies.

Hospice Visiting Hospice_Visiting

The Hospice Visiting program is for individuals faced with a life-threatening illness and for their families.

The Hospice philosophy is a life affirming one and is centered on empowering the client and the family as the key decision-makers in care.

Through in-home visits, specially trained volunteers provide support to individuals who have chosen to stay in the comfortable and familiar environment of their home.

Bereavement Support

Bereavement Services provide support to people experiencing grief and loss through the death of a loved one.

Acclaim Health offers the following Bereavement Services:

• In-home bereavement visits
• A Bereavement Resource Centre including a lending library
• Grief Information Handouts
• One-to-one support with a bereavement coordinator
• Support Groups
